- Transportation is based on your ridership choices on your electronic Transportation Form in the parent portal.
- Sewickley Academy Private bus routes and times will be sent to you 1-2 weeks before the first day of school.
- Public school bus routes and times are created by the public school districts, and they will communicate that information to families before the first day.

- Student Health Form—These forms are assigned to you as part of your enrollment checklist. If you have not done so, please log into your parent portal and submit the forms electronically.
- The supplemental forms below are in Tab 7 of the electronic Student Health Form. They need to be printed, completed, and returned to Beth Brown, the school nurse before the start of school.
